This is a manual I put together over the course of four months worth of research and construction. The Care Management Department asked me to help create a resource manual which covers all the counties where their patients are from. All of Piedmont Health's Care Managers now use this as their default resources manual.
This manual is for support services to all Piedmont Health patients, and for the public at large. It covers a four county-wide area, and covers such services as financial, legal, housing, providers, etc .... to make it easier for the public to find help above and beyond the scope of just a routine visit. It has been very popular, and many people have commented on how helpful it has been to them, as well as their family members.
